Transitional Care Management encompasses a range of services designed to ensure patients receive the necessary care and support as they transition from a hospital or medical facility to their home or another care setting. This period is crucial as patients are often vulnerable to complications, readmissions, and adverse events. TCM aims to provide a seamless transition by coordinating care, managing medications, scheduling follow-up appointments, and educating patients and their families about their health conditions and care plans.
A SuperBill is a detailed menu of charge codes used by healthcare providers to document and bill for services rendered during patient visits. The HybridChart SuperBill feature allows users to quickly generate and customize as needed. It includes all necessary information such as patient details, services provided, diagnosis codes, and charges. This streamlines the billing process, ensuring accuracy and efficiency in capturing all billable services.
An ADT feed is an admission, discharge, transfer feed from a facility such as a Hospital, Skilled Nursing Facility or Dialysis Center. This type of connection is a one-way push of information from a facility to HybridChart. The push of this information is triggered based on clinician name or National Provider Identifier (NPI). Most often, the facility pushes the patient information when a specified clinician is attached to the patient in the hospital’s EMR as either the admitting or consulting provider.
ADT feeds must be requested by the practice directly from the facility. Once an IT contact is identified at the facility, please instruct them to email us at interops@hybridchart.com with the following information:
  • Facility Name
  • Facility Address
  • Facility IT Contact Name
  • Practice Requesting Feed
  • Type of Connection
ADT feeds can simplify the process of adding a patient to a census in HybridChart. Typically, providers add patients by manually typing in a first name, last name, date of birth, select a location, and take a picture of facesheet. With an ADT feed, HybridChart receives an ADT message for a given patient, so providers can simply search for the last name, and automatically pull in relevant information.
While each facility configures their ADT feeds differently, the most common fields included are full name, date of birth, Medical Record Number (MRN), floor number and room number.
